Implantation bleeding: what it looks like and when to worry

What implantation bleeding is, when it happens, how it differs from a period, and when bleeding in early pregnancy needs urgent care.

· 3 min read · dauhieumangthai.com

Implantation bleeding is what many people search for when they notice a few unexpected drops of blood just before their period is due. Sometimes it is an early sign of pregnancy, but bleeding can also be a period or something that needs checking. This guide helps you recognise it and, more importantly, know when not to wait.

What implantation bleeding is

According to Mayo Clinic and the UK charity Tommy's (citing the NHS), implantation bleeding is a very small amount of blood that can appear as the embryo attaches to the lining of the womb. It can happen about 10 to 14 days after conception, around when your period would be due, sometimes with mild lower tummy cramps. Many pregnant people never have it, so no spotting does not mean no pregnancy.

Implantation bleeding versus a period

FeatureImplantation bleeding (if it happens)Period
AmountVery light, a few drops or streaks, not enough to soak a pad or underwearHeavier, needs pads
ColourLight pink or brownBright to dark red
PatternStays light, does not build upBuilds, then tapers over several days
DurationShort, usually no more than a few daysUsually three to seven days
Other signsPossibly mild crampsPeriod pain, sometimes clots

Everyone is different, so treat this as a rough guide. The reliable way to know is to test from the day your period is late, or a few days after the bleeding stops. See when to take a pregnancy test.

Bleeding when you know you are pregnant

Light bleeding in early pregnancy is not unusual, but do not try to judge it alone. Mayo Clinic's guidance for the first trimester:

  • Contact your doctor straight away for moderate to heavy bleeding, passing tissue, or bleeding with belly pain, cramping, fever or chills.
  • Contact them within 24 hours if bleeding lasts longer than a day.
  • Mention it at your next visit if it was light spotting that stopped within a day.
  • If your blood type is Rh negative, tell your doctor whenever you bleed.

From the second trimester, Mayo Clinic advises contacting your doctor right away for bleeding that lasts more than a few hours or comes with pain or contractions.

Emergency signs: do not wait

The NHS warns that an ectopic pregnancy, where the pregnancy grows outside the womb, usually in a fallopian tube, can cause:

  • Tummy pain low down on one side.
  • Vaginal bleeding, which may be watery and dark brown.
  • Pain in the tip of the shoulder.
  • Discomfort when peeing or pooing.

These usually appear between weeks 4 and 12 and can come before a positive test. If you have sudden severe pain, dizziness, fainting or look very pale, call emergency services (115 in Vietnam) or go to the nearest hospital now.

What to do next

  • Note when it started, how much, the colour and any other symptoms to tell your doctor.
  • Use pads rather than tampons so you can see how much you are bleeding.
  • Take a test if your period is late or the bleeding is unusual; our guide on how to use a pregnancy test helps you read it correctly.
  • Once positive, estimate your week with the due date calculator and book a check-up.

It is natural to feel frightened when you see blood. Try to stay calm, rest, and contact your doctor or the hospital's early pregnancy service for advice. Being checked quickly is never an overreaction: in many cases the scan is reassuring, and when it is not, early care makes a real difference.

Based on NHS and Mayo Clinic information; bleeding in pregnancy should always be assessed by a doctor.

Questions people ask

How long after implantation bleeding can I test?
Test from the day your period is late, or a few days after the bleeding stops. Testing too early can give a false negative because hCG is still low.
Is brown spotting in early pregnancy a problem?
A little brown spotting can happen, but you cannot assume it is fine. Tell your doctor, and go straight away if it is heavy, lasts more than a day, or comes with pain, one-sided pain or dizziness.
